Tip 2 diabetes mellituslu hastalarda sağlık okuryazarlığı düzeyinin tedaviye uyum ile ilişkisi

Abstract

Araştırma Tip 2 Diabetes Mellitus(DM)'lu hastalarda sağlık okuryazarlığı düzeyinin tedaviye uyum ile ilişkisini incelemek amacıyla tanımlayıcı olarak gerçekleştirildi. Araştırmanın evrenini 2018 yılında Kırklareli Devlet Hastanesi'ne en az bir yıllık Tip 2 DM tanısı ile başvuran 1214 hasta oluşturdu. Örneklem 292 olarak hesaplandı. Veriler, 01/07/2019-31/12/2019 tarihleri arasında anket formu, Yetişkin Sağlıkokuryazarlığı Ölçeği ve Tip 2 Diabetes Mellitus Tedavisinde Hasta Uyum Ölçeği ile yüz yüze toplandı. Veriler, SPSS Statistics 20 programında değerlendirildi. Tanımlayıcı istatistiklerde aritmetik ortalama±standart sapma, frekans ve yüzdelik dağılımlar verildi. Gruplar arası kıyaslamalarda Mann-Whitney U testi, Kruskal Wallis testi, Spearman Korelasyon Testi, Kendall Tau-b Korelasyonu testi, Pearson Korelasyon testi, Bağımsız Örneklem t-testi, Anova, Varyans analizi, Welch testi kullanıldı. Katılımcıların Yetişkin Sağlıkokuryazarlığı Ölçeği puan ortalamasının 13,13±4,23, Tip 2 Diabetes Mellitus Tedavisinde Hasta Uyum Ölçeği puan ortalamasının 74,22±12,13 olduğu bulundu. Yaşın, yaşanılan yerin, eğitim durumunun, medeni durumun, aile yapısının, mesleğin, gelir durumunun, sigara ve alkol kullanımının, evde kan şekeri ölçümü yapma durumunun, hastalığına uygun bir diyet önerilmesinin, önerilen diyete uyma durumunun, birlikte yaşadığı kişilerin verdiği desteğin, diyabet dışında herhangi bir kronik hastalık varlığının sağlık okuryazarlığı düzeyini etkilediği; yaşanılan yerin, ailedeki birey sayısının, gelir durumunun, diyabet tanısı konulduğu sürenin, evde kan şekeri ölçümü yapma durumunun, hastalığına uygun bir diyet önerilmesinin, önerilen diyete uyma durumunun, birlikte yaşadığı kişilerin verdiği desteğin tedaviye uyum düzeyini etkilediği bulundu. Tip 2 DM'li hastalarının sağlık okuryazarlığı düzeyleri artıkça tedaviye uyum düzeylerinin de anlamlı olarak artış gösterdiği saptandı.

The research was conducted descriptively in order to examine the relationship of health literacy levels with treatment adaptation in patients with Type 2 Diabetes Mellitus(DM). The population of the study consisted of 1214 patients who were diagnosed with Type 2 DM for at least a year applied to Kırklareli State Hospital in 2018. The sample was calculated as 292. Data were collected face-to-face with a questionnaire form, adult health literacy scale and patient compliance scale in the treatment of Type 2 Diabetes Mellitus between 01/07/2019 and 31/12/2019. Data were evaluated in SPSS Statistics 20 program. Arithmetic mean±standard deviation, frequency and percentile distributions were given in descriptive statistics. Mann Whitney U test, Kruskal-Wallis test, Spearman Correlation Test, Kendall Tau-b Correlation test, Pearson Correlation test, Independent Sample t-test, Anova, Variance analysis, Welch test were used in intergroup benchmarks. It was found that the mean adult health literacy scale score of the participants was 13.13±4.23, and the mean scale for adherence to the Type 2 DM treatment score was 74.22±12.13. Age, place of residence, education level, marital status, family structure, occupation, income status, smoking and alcohol use, measuring blood sugar at home, recommending a diet suitable for the disease, complying with the recommended diet, support provided by the people with whom they live, apart from diabetes the presence of any chronic disease affects the level of health literacy; it was found that the place of residence, the number of individuals in the family, the income status, the duration of the diagnosis of diabetes, the status of measuring blood sugar at home, the recommendation of a diet suitable for the disease, the adherence to the recommended diet, the support provided by the people with whom they live affect the level of compliance with the treatment. It was determined that as the health literacy levels of patients with Type 2 DM increased, their level of adherence to treatment increased significantly.
